Foster High School

If you are living in another person’s residence, we will need the following:

• Homeowner (LCISD resident) driver’s license with current address 
• Current land based utility bill – gas, water, or electric (not over 30 days old) in homeowner’s name showing service address
• Parent driver’s license, must match the homeowners address
• Any document mailed to the parent at the homeowner’s address within last 30 days
• Homeowner and parent must have the following forms filled out and notarized

1. Affidavit of Bona Fide Lamar CISD Residence
2. Parent Assurance of Dual Residency

INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED 

THESE FORMS AND DOCUMENTS WILL BE REQUIRED ON A YEARLY BASIS.
